We realize that visiting a new doctor for the first time can be stressful. Our office understands the anxiety a person feels when they are about to learn their way around, a new office, meet new people and receive care from a new doctor. Our aim is to remove as much stress and make you feel as comfortable as possible. To help minimize this hassle, we have three ways available for you to complete the necessary paperwork prior to your appointment. Fax, E-mail or from the website. We ask that you bring in the following information.
* Current Picture Identification (drivers license, passport etc) * List of any medications (prescription or over the counter) you are currently taking.
* Health Insurance Card.
Our office participates in most insurance programs. We ask that you provide us with the necessary information required to determine your coverage of benefits for chiropractic services.
